Transition & Family Support
For Students:
- EHCP reviews include transition planning from Year 9 onward.
- Access to college open days and supported visits.
For Parents:
- Guidance provided at annual reviews.
- Signposting to post-16 options and adult social care.
- Dedicated support from the Transition Lead and Family Support Worker.
- Preparation for Adulthood information booklet available for families.
